Required Forms for Student Enrollment

All students must submit a birth certificate, baptismal certificate, immunization form, social security card, and emergency card by August 1st to enroll in St. Joseph Catholic School.

MEDICAL:
All students who use an EpiPen or who will receive any medication while at school must complete the following forms by August 1, 2023.

Only new students in grades 5-8 who will be playing sports need to complete the MSHAA Sports Physical form.
